Medicare ACCESS MSK Track: Every Company Approved for Chronic Pain Care

The musculoskeletal track of Medicare's ACCESS model is for people with Original Medicare living with chronic musculoskeletal pain, which Medicare describes as ongoing muscle or joint pain. CMS lists 74 organizations approved for this track as of its October 20, 2026 update, 7 of them for this track alone.

What this track pays for

ACCESS pays each organization a recurring amount for every patient it manages in the track, and full payment depends on the share of its patients who improve on validated, patient reported measures of pain and function. CMS sets the measures and each patient's target based on where they start. CMS lets programs combine clinician visits, exercise and lifestyle support, apps and FDA authorized devices or software, delivered in person or virtually.

How to join a program in this track

If you have Original Medicare, start at Medicare.gov/ACCESS, which has a tool to find participating care organizations. You contact the organization directly to sign up, with or without a referral. Medicare says most organizations charge $0 to $7 a month. Your own doctor can stay involved, receive progress updates and bill Medicare for reviewing your care.

Does Medicare Advantage cover ACCESS programs?

No. Medicare says people with a Medicare Advantage plan cannot use ACCESS, though their plan may offer something similar. Several major health plans have pledged to offer an ACCESS aligned payment option, so ask your plan directly.
