CombineHealth
A Y Combinator backed startup selling AI agents, named Amy, Mark, Adam, Rachel and Taylor, that code, bill, chase denials and analyze claims for US health systems. Founded in San Francisco in 2022, it has grown to roughly a dozen employees on two continents, but it has not yet surfaced a named hospital customer or a public funding filing.
A thousand charts, fifteen days
For fifteen days, CombineHealth split 1,000 real emergency department charts with the professional coding team at Edelberg & Associates, an outside medical coding audit firm. About 100 charts a day went to each side: CombineHealth’s automated coding agent, which it calls Amy, on one, Edelberg’s expert human coders on the other. Edelberg’s own auditors then graded both sets of codes against documentation support, coding guidelines, specificity, sequencing, medical necessity and payer requirements, rather than simply comparing the AI’s answers to the humans’ answers.
By CombineHealth’s own account, Amy matched or beat the human coders on every measured dimension, coding about 85 percent of the charts without a person in the loop and flagging the rest for review. It is the single most concrete, outside audited result in the company’s public record, though it is a test CombineHealth designed and published itself.
The problem it is chasing
CombineHealth frames the problem in blunt numbers on its own site: the average healthcare organization loses $3 million a year to revenue cycle inefficiencies, 82 percent of providers say they struggle to find where their revenue cycle breaks down, and only 45 percent say they have full visibility into it. Those figures are company stated and were not independently checked for this profile.
Medical coding, billing and denial management are the kind of repetitive, rules heavy, high volume paperwork that large language models are being pointed at across healthcare, and revenue cycle management is one of the more crowded corners of that push. Tracxn ranks CombineHealth 69th among 330 active competitors it tracks in the category, well behind better funded rivals such as Fathom, RapidClaims and Charta Health.
Two founders, two earlier startups
CombineHealth was founded by Sourabh Agrawal and Shikha Mohanty in 2022, according to the company’s own about page and to Tracxn’s independently compiled profile, which agree on both names and the year. Agrawal is CEO. His biography on Y Combinator’s own company page says he spent eight years building and deploying machine learning models before CombineHealth: as an AI consultant at Bosch working on autonomous driving, then as a quant associate at Goldman Sachs, after studying electrical engineering at IIT Bombay.
He had already co-founded two other startups by then. Insane.ai, an AI powered fitness app described on the same YC page, was seed funded, scaled to more than 150,000 users and grew to a team of more than 20. UpTrain AI, an open source LLMOps company built with a co-founder identified in its own launch post only as “Shikha,” went through Y Combinator and describes itself as enterprise tooling to evaluate, monitor and test large language model applications, open source under Apache 2.0 with more than 1,700 GitHub stars by its own account. That earlier venture is presumably the same Shikha Mohanty who co-founded CombineHealth, though the source that names her only by first name does not confirm the surname. Tracxn credits Mohanty as founder of one other company and an angel investor in a further startup, without naming either, and independent detail on her background beyond that is thin.
From a San Francisco seed round to a Bangalore desk
CombineHealth says on its about page that it is “backed by leading Silicon Valley investors including Y Combinator,” without stating a dollar figure. Two independent data aggregators put rough numbers on that: aVenture records a $500,000 round completed by April 5, 2023 and a cumulative $1,000,000 raised by that date, and Tracxn separately counts one institutional investor, unnamed. A search of SEC EDGAR, by company name and by full text, turns up no Form D filed by any entity called CombineHealth or Combine Health as of September 25, 2026, so neither aggregator figure is confirmed by a public securities filing.
Headcount has grown since. aVenture counted 8 employees in February 2024. A Bangalore, India job listing posted in mid-2026 described CombineHealth as a 12-person company hiring its first local full stack engineer. Tracxn put the total at 14 employees as of July 31, 2026. The company now sells a five-agent lineup, Amy for coding, Mark for billing, Adam for denial management, Rachel for appeals and Taylor for analytics, and its homepage says its AI agents are “live” with customers it does not name.

Read plainly: the strongest evidence CombineHealth has put in public view is the Edelberg audited coding test, and even that is a study CombineHealth designed and published itself, with an outside firm grading both sides. Who is actually paying for the platform today, at what scale and under what contract, remains unstated.
What to watch
- Whether CombineHealth or an investor ever files a Form D, which would put a confirmed number and named investors on the aVenture and Tracxn estimates.
- A named hospital or health system customer, rather than the unnamed testimonials on the homepage and in the ED coding case study.
- Which EHRs and practice management systems the platform actually connects to; the homepage promises “Integration with Major EHRs and PMSs” without naming any.
- Growth of the Bangalore, India engineering team, which was still hiring its first local full stack engineer as of mid-2026.
