CoreCare
A Y Combinator backed platform that automates eligibility, census and Medicaid workflows for skilled nursing facilities. Five and a half years after its first customer, it has sold its newest product line to a larger admissions platform and narrowed back to what it calls revenue integrity.
A company built for the spreadsheet, not the ribbon cutting
Dennis Antonelos started CoreCare in January 2020, had a product running by March, and was accepted into Y Combinator’s Summer 2020 batch in April, as nursing homes became the center of a national health emergency. By October, TechCrunch reported a $3 million seed round from Primetime Partners, Goat Capital, FundersClub and Liquid2Ventures, with a dozen customers already signed in Texas.
The product was not glamorous. CoreCare linked electronic health records to Medicare and Medicaid eligibility systems and payer portals, and automated the billing and revenue cycle paperwork that skilled nursing facilities (SNFs) do by hand. “We’re going through the organization and eliminating administrative waste so the organization can invest newly found resources into patient care,” Antonelos told TechCrunch at the time.
One early customer was Creative Solutions in Healthcare (CSNHC), a family owned Fort Worth, Texas operator whose president, Gary Blake, called early results outstanding in that same article. That relationship has run more than five years and recurs in later releases.
The problem: a payment system that runs on paper
Skilled nursing is a low margin, high paperwork business. Facilities bill Medicare, Medicaid and private payers under rules that vary by state, and a missed eligibility check or unsubmitted state form can become a write off months later. CoreCare’s pitch, consistent across its releases, is that this work still ran on spreadsheets and manual review, and that connecting those systems and flagging problems before a claim is submitted protects revenue otherwise lost.
Its 2024 boilerplate claimed operators using CoreCare could eliminate up to 95 percent of billing errors before submission and cut days sales outstanding by up to 50 percent. Those are company claims, not audited figures.
Founders
Antonelos is CEO and one of two founders listed on Y Combinator’s page. Per a bio published by The Org, he previously held roles at Arcus, Drone USA, Xoriant and Pike Falls Corporation, and studied at Fairfield University, the Questrom School of Business at Boston University, and an executive program at Harvard Business School. His YC founder bio reads: “Always running towards the big, complicated problems.”
Mehrdad Shafaie is the other listed founder, titled Founder and COO in company materials. His YC bio says he is drawn to “really hard problems in payments and money movement.” A third executive, Geoffrey Roguelon, is CTO on CoreCare’s about page but is not listed as a founder on Y Combinator.
From a single feature to a platform, and back again
CoreCare grew by adding modules. Eligibility automation reached Ohio in 2022 with United Church Homes. A clinical reimbursement tool for the new Minimum Data Set 3.0 rules followed in 2024 with CSNHC. In November 2024 came CoreCare Admit, a tracker for Preadmission Screening and Resident Review (PASRR) compliance, referenced by Legacy Health Services. In 2025 it moved into referral intake with Pre-Admit, adopted by CSNHC, Focused Post Acute Care Partners and Senior Living Properties Operations across dozens of facilities.
In April 2026 CoreCare gave Pre-Admit up. Exacare ai, a larger admissions platform serving more than 2,000 facilities, announced it was welcoming CoreCare’s Pre-Admit customers into its own product. “This was a deliberate move to ensure our Pre-Admit customers are well served,” Antonelos said. “We’re focused on our core AI-powered revenue integrity platform and executing a clean transition.” CoreCare’s site now describes the company only as “the AI Platform for Proactive Revenue Integrity for LTC,” built around census integrity, eligibility monitoring and Medicaid pending and renewal tracking.

Read plainly: CoreCare has a repeatable customer story, the same Texas operator that signed on in its first year is still a reference customer in 2026, and independent trade coverage confirms at least one deployment. What is not independently verified is scale. Every facility count, revenue figure and error-reduction number comes from the company itself, and two basic facts, who founded it and when, are reported differently by different data providers.
What to watch
- Whether CoreCare discloses terms of the Pre-Admit transition to Exacare ai, including whether it received cash, equity or neither.
- A funding announcement that names investors and an amount for the two 2024 financing events PitchBook lists without detail.
- Whether a Form D or other public filing ever appears for CoreCare, Inc. in SEC EDGAR; none was found as of this profile.
- Independent, audited figures for the facility count, revenue and error reduction claims the company currently publishes.
