Attunement
A San Francisco startup trying to turn scattered behavioral health paperwork, faxes and EHR exports into structured, usable data. It has rebuilt its product at least twice since a January 2024 Y Combinator launch and now describes itself as clinical records infrastructure for specialty medicine.
A slogan that keeps changing
Attunement’s own website has described three different products in about two years. At launch in January 2024, the pitch was passive outcome tracking: “we’re building a patient monitoring and treatment recommendation platform for behavioral health,” combining chat-based mental health detection, physiological biomarkers and self-reported check-ins so a provider would not have to rely on “I just know” to judge whether a patient was improving.
By early 2025, an archived version of the site described a different business entirely: AttuneOS, a service that staffs a “testing clinic” with an outsourced, HIPAA-trained “Digital Assistant” handling scheduling, consultations and converting reports to charts. A founding engineer job posting from the same period described the company as building “compliance infrastructure for AI in behavioral health,” with data pipelines connecting to EHRs over FHIR and HL7.
The current site, live as of September 2026, drops behavioral health from the headline altogether: “Turn fragmented clinical records into clinical clarity,” helping “specialty medicine and healthcare organizations turn outside records, EHR data, faxes, and clinical imaging into structured, clinician-ready intelligence.” The underlying problem, disorganized outside records, has stayed constant even as the product built around it has not.
The problem
The original YC launch post framed the gap this way: behavioral health providers rarely use standardized tools like the PHQ-9 depression scale consistently, so progress is judged by feel. Medicaid spends roughly $110 billion a year on behavioral health services, the post said, with little way to measure whether that money is producing better outcomes.
The current framing is narrower and more operational: clinical records arrive at a practice in whatever format the last provider used, a fax, a scanned PDF, an EHR export, and someone has to turn that into something a clinician or an AI system can actually use. That is a data-plumbing problem shared across specialty medicine, not one specific to therapy or psychiatry.
Founders
Angie Muller is CEO and, as of the current Y Combinator listing, the only founder Y Combinator names. Before Attunement, she led partnerships at Stripe and Nvidia, working on seed venture relationships, synthetic data and data labeling and annotation, according to a CB Insights summary of her own bio. She has a BA in psychology from Barnard College, Columbia University, and did graduate research in applied neuroscience at the Institute of Psychiatry, Psychology and Neuroscience at King’s College London.
Briar Smith co-founded the company as CTO. He is a University of Waterloo mechatronics engineering graduate who founded WATonomous, a student self-driving-vehicle research team sponsored by General Motors and SAE, and previously worked on self-driving research at Lyft and as a founding engineer at Tab, a personal-AI wearable device startup. His own Forbes Technology Council bio, current as of when it was fetched, lists him as “currently” co-founder and CEO of NeuraWell Therapeutics, a clinical-stage psychiatric drug developer, and describes Attunement in the past tense as “a remote monitoring platform for behavioral health.” Y Combinator’s current company page for Attunement lists only Angie Muller as a founder and puts the team at three people, which is consistent with Smith having moved on.
Timeline
Attunement went through Y Combinator’s Winter 2024 batch and launched publicly in January 2024. By 2025, per an archived product page, it was selling an outsourced clinic-staffing product rather than software alone. In October 2025, Muller was still personally recruiting a founding engineer on LinkedIn, describing the company as “early-stage” with “deep clinical roots,” aiming to make “behavioral-health documentation objective, scalable, and trusted.” The company’s public messaging had shifted again to records structuring by the time its live site was checked in September 2026.

Read plainly: what is documented is that Attunement went through Y Combinator in 2024 and is still operating in some form under Angie Muller in 2026. Everything about what the product does, who runs it day to day, and who uses it has to be taken on the company’s own word, because no regulator, court, grant agency or named customer has put anything about Attunement on the public record.
What to watch
- Whether the company settles on one product description, or keeps rebuilding around the same underlying records problem.
- Any named provider or payer customer, which would be the first independent confirmation the product is in use.
- Whether Briar Smith’s status is formalized anywhere, and whether Attunement replaces him with another named co-founder or technical lead.
- Any FDA filing, EHR integration announcement, or funding disclosure beyond the Y Combinator round.
