mdhub
An AI native operating system that replaces the EHR, CRM and billing stack a behavioral health clinic normally stitches together from five vendors. Founded in 2023, it has grown from a single AI scribe to a full platform now used, by the company's own count, by more than 10,000 clinicians.
Five systems, one front desk
The problem mdhub describes is familiar to anyone who has worked a front desk at a mental health clinic: scheduling in one tool, documentation in another, billing in a third, admissions in a fourth, patient texting in a fifth, none talking to each other. On a podcast interview in 2026, founder Dominik Middelmann put it plainly: patients all call on Monday at 8 a.m., and no matter how many people a clinic staffs, some calls get missed.
mdhub's answer is to give the clinic three named AI workers inside one connected system instead of five disconnected ones. Sarah answers calls, texts and faxes, verifies insurance and books the patient. Emma listens to the clinical session and drafts the note, the CPT codes and the treatment plan update. Eric takes the finished note, builds the claim and submits it same day. The company says Sarah has booked more than 100,000 patients into care and that Emma has sat in on more than 3 million patient sessions across more than 1 million patients, saving clinicians upward of 500,000 hours of admin work.
Why behavioral health specifically
The case for specialization shows up in the numbers mdhub and its customers cite. Talkiatry, one of the larger in network telepsychiatry groups, told mdhub that 45 percent of clinicians go out of network with insurers because reimbursement is low and paperwork is heavy, a claim mdhub attributes to Talkiatry's own data. The National Institute of Mental Health estimate mdhub cites, that 60 percent of US adults with a diagnosable mental illness go untreated in a given year, sets the stakes.
General purpose AI scribes, trained mostly on primary care and surgical encounters, are mdhub's foil. A documentation tool trained on psychiatric intake formats, SOAP and DAP notes, and therapy language needs less editing after the fact, the pitch goes, than a generic tool dropped into a behavioral health setting.
Two operators, eight years in together
Dominik Middelmann and Efrén Lamolda are not first time collaborators. Both worked together starting in 2018 at what Middelmann has described as a hyperscale start up in an unrelated industry, where Lamolda was a founding engineer and Middelmann an early operator; Middelmann says that company scaled from zero to 4,000 employees and past 300 million dollars in revenue. Middelmann holds a Masters in Management from London Business School and started at BCG. Lamolda holds an MSc in computer science.
By Middelmann's account on the Outcomes Rocket podcast, the idea for mdhub started when Lamolda, already working on a side project with a psychiatrist, told him the software that psychiatrist and his peers used looked terrible and that they should build something better. Middelmann says his interest in behavioral health had deepened during the COVID years, when colleagues, family and friends around him were struggling with their mental health.
From scribe to full stack
mdhub's first product, launched through Y Combinator's Summer 2024 batch, was a clinical AI assistant for documentation, aimed at solo and small group psychiatric and therapy practices. The company later brought BD Emerson CPA in to run a SOC 2 readiness review and a HIPAA reassessment, completing a SOC 2 Type I audit; the auditor's case study, published in 2026, describes mdhub as ready for the longer SOC 2 Type II review. By September 2026 mdhub's own site claimed SOC 2 Type II certification.
In April 2026, mdhub announced its full platform, adding an EHR, a CRM it calls an access management layer, and revenue cycle management to the existing AI workers, positioning itself as a replacement for the whole stack rather than a point solution bolted onto an incumbent EHR. Talkiatry rolled the Clinical AI Assistant out across its psychiatrists and therapists around the same period. Other named customers include Elite DNA Behavioral Health, which says it used Sarah to scale admissions across more than 40 locations, and Central Valley Family Therapy, which says it grew past 35 clinicians without growing its front desk.

Read plainly: mdhub has a real, named customer base and a documented security audit trail, which is more than many early stage clinical AI vendors can show. What it does not have is any outcome data, peer reviewed or otherwise, on whether its notes, its scheduling or its billing actually improve care, and its funding numbers rest on private trackers rather than a public filing.
What to watch
- Whether a Form D or other SEC filing appears for mdhub as it raises beyond its Y Combinator round.
- Independent confirmation of the SOC 2 Type II certification the company claims on its site.
- Whether mdhub names additional large customers beyond Talkiatry, Elite DNA and Central Valley Family Therapy, and whether it publishes usage or outcome data with any third party review.
- How the EHR, CRM and RCM products launched in April 2026 perform once clinics run a full billing cycle through them, not just documentation.
