Hippo Scribe

An ambient AI scribe built for physical, occupational and speech language therapists, not doctors. It records the session, drafts the note in the clinician's own format, and pushes it into whatever EMR the practice already runs.

A therapist named Marissa

In its Y Combinator company profile, Hippo Scribe introduces its customer with a composite sketch. Meet Marissa, a pediatric speech language pathologist who sees eight children a day and spends her energy keeping each one engaged. Meet John too, a physical therapist who treats 8 to 13 patients a day, helping adults recover from pain and older patients avoid falls. Both go home and spend one to two hours writing down what just happened in each session, because there was no time to write anything down while it was happening.

That gap between the session and the note is what Hippo Scribe is built to close. The product records the visit, or a clinician's dictation afterward, and generates a first draft of the clinical note within seconds of the recording ending. A Chrome extension places that draft next to whatever web based EMR the practice uses, so the clinician can review it and copy it in rather than typing from scratch.

The problem it names

Hippo Scribe's own framing of the problem is specific: burdensome documentation is, in its words, the number two reason causing burnout for physical therapists, and more than half of physical therapists write documentation in their free time without pay, up to two hours a day. The company has not published the study behind that figure, so it should be read as the company's own framing of the problem, not an independently verified statistic.

The company's blog describes the same problem from the pediatric side. One customer told Hippo Scribe, “Doing point of care documentation is almost impossible with our line of work.” Another described the emotional cost of the job itself: “We have a lot of autistic clients and young ones... I love it, but it definitely, like, you have to give a lot of your, like, self. I guess you need a little extra time to just chill and recharge again.”

The founder

Hippo Scribe's public founder is Yansen Zhou. In his Y Combinator launch post he introduced himself this way: “Hello world, I’m Yansen, son of clinicians and I’m building AI solutions to make documentation easier.” Beyond that line, neither the YC page nor the company's own materials describe his clinician relatives in more detail.

Zhou's own website lists a software career before Hippo Scribe: he says he built products at Snapchat, on the Discover feed, and at Airbnb, on payments processing. A 2020 SEC filing, Airbnb's Form S-8 registering shares for current and former employees, lists a Zhou Yansen among the several thousand people covered by that plan, which is consistent with Zhou's own account of working there. Hippo Scribe's mobile app and Chrome extension are both published under the name Icarus Lab Inc., a Delaware entity with a registered address in Dover, according to both the Apple App Store and the Chrome Web Store listings.

Where things stand

Hippo Scribe launched twice on Y Combinator's platform: once around its Winter 2023 batch, and again with a later post titled “Free AI scribe for PT/OT/SLP,” which introduced a free plan and pointed to a Facebook group where, the company said, “600+ clinicians are discussing AI Scribe.” The Chrome extension has continued to receive updates, with a build dated May 17, 2026 on the Chrome Web Store, and the company added a companion iPad and iPhone app, Hippo Scribe Go, copyrighted 2024 to Icarus Lab Inc.

Zhou's own site, which is dated to 2026, describes Hippo Scribe in the past tense: “Previously, I founded Hippo Scribe, a profitable AI scribe startup.” The same page says he is “currently a Member of Technical Staff at OpenAI.” That leaves an open question the public record does not answer: whether Zhou still runs Hippo Scribe day to day, or has handed it off while the product itself keeps shipping updates.

What is proven, and what is still claimed

Operations track. Hippo Scribe is a documentation tool, not a medical device or a drug, so there is no FDA clearance, clinical trial registry or NIH grant to check it against. What can be checked is who built it, what platforms verify about it, and what its own customers say on the record.

Read plainly: Hippo Scribe is a real, actively maintained product built and run under a named Delaware company, with a founder whose earlier career at Airbnb and Snapchat checks out against an independent SEC filing. What has not been independently verified is scale. Clinician counts, the burnout statistics the company opens with, and the claim of profitability all come from the company or its founder, with no outside audit or press report behind them.

What to watch

  • Whether Yansen Zhou, now describing himself as a Member of Technical Staff at OpenAI, remains actively involved in running Hippo Scribe, or whether new leadership emerges publicly.
  • Any named EMR partnership or marketplace listing, such as with WebPT, that would move the company's integration claims from company-stated to independently checkable.
  • Whether the company ever files a Form D or announces an institutional round, which would be the first public financing record for the business.
  • Independent reviews or case studies from pediatric OT/SLP or PT practices beyond the testimonials the company has chosen to publish.
