Mederva
A San Francisco startup that pairs health coaching with licensed clinicians and bills the work through a patient's own insurance, aiming to turn a doctor's fifteen minute visit into a continuous, data-driven protocol between visits.
A year of drifting labs
Emily had been dragging through her afternoons for over a year, according to a case study Mederva posts on its own site. Her labs kept moving the wrong way and her doctor kept adding medications. Mederva says it pulled her records, flagged root causes she had not been told about, and built a coach-led protocol around the three changes it judged would matter most. The company reports daily habits, weekly check-ins, and a plan that adjusted as she improved. No outside verification of Emily's case, or even her existence as a real patient rather than a composite, was found.
The gap between visits
Mederva's pitch starts with a complaint many primary care patients would recognize. “Your PCP has 15 minutes and one visit’s worth of context,” the company says on its site. Its answer is to connect a patient's existing medical records, labs, and other health data, build a protocol around the highest impact changes, and hand the patient a coach who checks in between doctor visits. The company positions itself against direct to consumer lab testing services, drawing a specific contrast with Function Health: “Function shows you labs and general guidance for $499/year. We pull data that already exists, build a protocol around the changes that matter most, and pair you with a coach who walks you through it. Covered by insurance.”
That last point, insurance coverage, is the structural bet. Mederva's terms of service split its offering into two pieces: “Remote Care,” delivered by licensed physicians, registered dietitians, physician assistants, nurse practitioners, and behavioral health therapists, and separate “Wellness Services,” where the terms specify that Mederva's health coaches “are notClinicians and are not providing Remote Care or any other professional services requiring professional licensure.” That distinction lets the coaching sit alongside licensed care without every coach needing a medical license, while the clinical half of the visit can be billed.
Three founders, one basketball court
Mederva's YC page lists two active founders. One studied cognitive psychology and mechanical engineering, focused on human factors, at Tufts, and says he was “the first designer at Ginger (now Headspace Health)” before more than 20 years in product and UX design; that founder is Sabih Mir, per Y Combinator's own March 2022 welcome post and Mederva's Crunchbase listing. The other studied computer science and medicine, trained at Stanford in musculoskeletal radiology, joined the faculty there, and built a top ranked iOS health app with more than 5 million users before starting Mederva; a March 2026 conference bio names him as Dr. Mohammed Kaleel. Kaleel's YC bio says the founders “met my cofounders playing basketball in the Mission in SF.”
A third name shows up only at the company's start. Y Combinator's own welcome tweet from March 22, 2022 reads: “Welcome to YC W22, @sabihmir, @mkaleel, Billy Rinchin, and team @mederva!” Billy Rinchin does not appear among the active founders on Mederva's current YC page, and no source found explains when or why he left the visible founding team.
A pivot from telehealth to insurance billed coaching
The company that entered YC in 2022 was not selling longevity coaching. Y Combinator's own description of the batch company reads: “Mederva Health makes it possible for doctors in developing countries to provide better care for their patients by tapping into the best US medical experts.” A contemporaneous Tracxn listing describes the same company as “a digital platform for doctors to manage consultations and patient's health records,” with a continuing education offering attached. That is a cross border medical consultation business, aimed at doctors, not patients.
By the time of this research, that description had been replaced entirely. Mederva now sells directly to patients through primary care clinics, bills the clinical portion of the work under those clinics' insurance contracts, and describes its own scale in a job listing, captured by the Wayback Machine, for a care management billing role: “We run a fast-growing care management program billed under partner clinic TINs across a mixed payer population,” the listing says, aiming to grow “from ~24 clinics to 100+.” No public announcement of the pivot itself, its timing, or its reasons was found.

Read plainly: nothing here has gone through the FDA, a registered trial, or a peer-reviewed journal, because Mederva is not selling a device or a drug. It is selling a service model, coaching layered onto licensed clinical care and billed through existing insurance codes, and the evidence that matters most for that model is billing and retention data the company has not published. What is on the record is a real clinical structure, a real pivot from an earlier telehealth idea, and several small disagreements between the company's own account and the trackers that follow it.
What to watch
- Whether Mederva files a Form D of its own with the SEC, separate from the Sydecar administered vehicle that has filed on its behalf.
- Which of Sabih Mir or Mohammed Kaleel holds the CEO title going forward, and whether Mederva clarifies it publicly.
- Any published account of what happened to Billy Rinchin's role after the 2022 YC batch.
- Whether Mederva discloses clinic count, payer mix, or outcomes data beyond the single case study on its site.
