Andy AI

An AI ambient scribe built to turn a home health nurse's recorded visit into a completed OASIS chart. Founded in 2023, out of Y Combinator's Winter 2024 batch, with a small companion app now live on Google Play.

One tap, one visit

A home health nurse opens the Andy companion app, records the patient visit, and captures any images she needs. The audio and images upload to what the company describes as a HIPAA compliant cloud. From there, the agency's electronic health record connects to Andy to receive the completed note. The app itself does not generate the OASIS assessment on the device, according to its Google Play listing; that happens once the agency has connected its EHR to Andy's cloud.

The company's website puts the pitch plainly: “No more tedious back-and-forths. Andy’s all-in-one scribing, QA and coding delivers an accurate and defensible note every time.”

The paperwork problem

Home health is, in the company's own framing, the fastest growing segment of healthcare, on track to double to $251 billion by 2031. Growth has run into a staffing problem: a wave of nursing burnout, much of it tied to charting. A single home visit's documentation, the company says, can take 90 minutes, and it estimates the resulting inefficiency costs home health agencies $8 billion a year industrywide.

The regulatory anchor for that paperwork is OASIS, the Outcome and Assessment Information Set that Medicare-certified home health agencies must complete for every patient. It is long, detailed, and tied directly to how an agency gets paid, which is why a tool that shortens it also touches agency revenue, not just clinician time.

Two founders from Google and Apple

Tiantian Zha, the CEO and co-founder, studied computer science at Princeton and spent roughly eight years at Google, including as a product manager on Google's healthcare business, Verily (Google Life Sciences). There she was the first product manager on Debug, an AI and robotics project aimed at reducing mosquito-borne disease. In a podcast interview with MarketScale, Zha discussed the difference between traditional software and AI in home health and Andy's work getting OASIS forms done in under 15 minutes.

Max Akhterov, the CTO and co-founder, holds a Ph.D. in physics from UC Irvine. In a 2020 interview with the UC Irvine School of Physical Sciences, while working as a senior machine learning engineer at Apple, he described his focus this way: “I’m passionate about designing new sensing platforms for consumer-centered diagnostics. Currently, I’m developing algorithms for health sensors on the Apple Watch.” Company materials credit him with work on the Apple Watch's passive atrial fibrillation detection, which by the company's account runs on tens of millions of devices.

From Y Combinator to a live app

Andy AI launched out of Y Combinator's Winter 2024 batch, with a company page listing a team size of 5 and Gustaf Alstromer as its primary YC partner. It is also listed as a South Park Commons portfolio company. A pre-seed round was reported in late February 2024, and about six weeks later, in April 2024, a small investor vehicle organized around the company filed with the SEC. By September 2026, Andy AI's companion Android app, built for clinicians to record visits and upload data, showed more than 500 downloads on Google Play, and had been updated as recently as that month.

What is proven, and what is still claimed

Operations track. Andy AI sells software, not a medical device or a drug, so there is no FDA clearance, registered clinical trial, or NIH award to check it against. For a company like this, the record runs through funding filings, accelerator rosters, app store listings, and named customers, several of which have not been made public.

Outcome metricsCompany claims include up to 45% more detailed charts, 2x nurse productivity, visits to export in under 5 days, and a 7% higher case-mix weight for agencies using Andy. No independent study of these figures was found.Company-stated

Read plainly: Andy AI has a real product, a working app with a small but growing download count, an accelerator pedigree, and a documented investor vehicle, but the core claims about accuracy, productivity gains and case-mix improvement all come from the company itself. There is no named customer, no independent audit and no disclosed EHR partnership to check those claims against yet.

What to watch

  • Whether Andy AI, Inc. ever files a Form D under its own name, which would show the size of a direct raise rather than an SPV built around it.
  • Named home health agency customers or a disclosed EHR integration partner such as Homecare Homebase, WellSky or Axxess.
  • Independent validation of the stated productivity, case-mix and charting metrics.
  • Growth in app downloads and any update to the GetLatka revenue estimate.
