A San Francisco company that built its business running GLP-1 telehealth brands like Fella Health and Bolt Pharmacy, and in 2026 started pitching itself as something else: the first vertically integrated AI doctor in Europe.
Two homepages, five years apart
Archived copies of aiosmedical.com from 2025 show a company describing itself as plumbing: an “AI operating system for consumer healthcare,” built to run other people’s brands with near zero marginal human time. By mid 2026 the live site had been reduced to a single dark screen carrying one line: “The first vertically integrated AI doctor in Europe.”
That is a different company on paper, even if the underlying technology and team overlap. The Y Combinator listing for the business, still live under the same description as of this writing, keeps the older framing: an infrastructure layer that has “started by building this infrastructure to power our own brands - including Bolt Pharmacy and Fella Health, among others.” with a stated goal of serving 100 million patients by 2035 and becoming “the world’s first trillion-dollar healthcare company.”
Where the money actually comes from
The clearest paying business is Fella Health, a telemedicine clinic that prescribes GLP-1 medications, semaglutide and tirzepatide, along with testosterone therapy and coaching, to men with obesity. It launched in the US in June 2021, according to the company’s own LinkedIn page, and now also runs a UK version plus a sister brand for women, Delilah Health, and a UK dispensing arm, Bolt Pharmacy.
Bolt Pharmacy is the one part of the business with a hard regulatory footprint. Its website names a superintendent pharmacist and a medical lead by General Pharmaceutical Council and General Medical Council registration numbers, standard requirements for a UK online pharmacy dispensing prescription-only medicine. Its consumer app, on both the Apple App Store and Google Play, lists the publisher as Aios Inc., confirming that Bolt and AIOS share a corporate parent even though the two brands market separately.
That listing gives Aios Inc. an Austin, Texas address, matching PitchBook’s corporate office field for Fella. The company’s public identity, on Y Combinator, LinkedIn and its past homepage, is San Francisco. The two addresses do not match.
A founder who has already built and sold one AI company
Richie Cartwright is a Cambridge graduate who, per his Y Combinator bio, worked in tech and media roles before Entrepreneur First and founding FlowX, a computer vision company reading UK traffic cameras to ease congestion. FlowX won a $650,000 UK Department for Transport contract and was later acquired, per Cartwright and independent founder profiles, by Vivacity Labs.
He has said the motivation for what came next was personal. According to his Y Combinator bio, the business was “inspired by my own 6-year secret battle with binge eating disorder.” Fella Health, the men’s obesity clinic he co-founded with Luke Harries, followed that experience directly: a weight loss product aimed at a group, men, that the industry had mostly ignored.
From bootstrapped clinic to accelerator pitch
Multiple sources agree the company started small. BrandProject states it “led Fella’s (AIOS) seed round in 2021,” without naming an amount. PitchBook records a $75,000 seed closing on October 7, 2021, with $225,000 raised in total and ten investors including BrandProject, F6S, Fitt Capital, Global Founders Capital and Interact Software. A separate market-analytics profile puts the seed at $130,000 from Y Combinator, Global Founders Capital, AngelList and angels who had founded Indeed, Curative and Alan. The three figures do not agree, and none ties to a filed SEC Form D, since none turns up under either the AIOS Medical or Fella Health name in EDGAR’s own company search.
What is not in dispute: the company kept operating on little outside capital while GLP-1 demand exploded. By 2026, a market-analytics profile tracking Fella Health described it as cashflow positive with more than $50 million in annual recurring revenue, an estimate this page cannot independently confirm.
Then came HF0. On June 24, 2026, an attendee at the HF0 residency’s demo day posted that Cartwright was showing an “AI Doctor that already is seeing significant sales in Europe,” drawing investors to his booth. A tech aggregator that republished the post headlined it with a $350 million ARR target, a figure not found stated anywhere else.

Read plainly: the operations evidence is real but narrow. A UK pharmacy license and matching app store records confirm AIOS runs a real dispensing business with named, regulated staff. Most of what is said about money, headcount and history comes from data aggregators and company statements that disagree with each other, with no filing to settle them. The 2026 pivot toward an “AI doctor” product rests on one eyewitness account of a demo, not a launch, a release or a published result.
What to watch
- Whether AIOS publishes any detail on the AI doctor product shown at HF0, including what it actually automates and where in Europe it has paying users.
- Whether a Form D or other funding filing ever appears for AIOS Medical, Fella Health, or a related entity, which would settle the seed amount question.
- Whether the company reconciles its San Francisco identity with the Austin, Texas corporate address on file with Apple and Google.
- Independent confirmation of the $50 million ARR and $350 million ARR target figures currently sourced only to a data aggregator and a conference tweet.
